On November 6, 2021, an employee was using a wrench to release pressure from a carbon dioxide supply pipe. The pipe had been venting and it suddenly stopped. As the employee was checking the pipe, it disconnected and smashed his right index, middle, and ring fingers. The employee suffered bruises too all three fingers and an amputated middle fingertip.

On August 31, 2021, an employee was running an extruder. As the employee used a metal spatula to scrape built-up extrudate from the chill knife, he lost his balance and his little finger was caught in the rollers. The fingertip was degloved.

On July 24, 2021, at 4:15 p.m., an employee and a co-worker were verifying prior work done on a machine. The employee had his hand in the freezer bar sealer area when the co-worker activated the machine by opening the air valve that powered the sealer. The sealer then closed on the employee's right hand, resulting in an open fracture and an amputation of the middle fingertip.
